What to Expect
During your first visit, the doctor will go over the information you have provided in your new patient paperwork. For diagnosis, they will ask questions, perform tests and take x-rays if necessary. The doctor will share openly and honestly the services we provide, and together with you, devise a plan that best meets your needs to improve your health as quickly as possible.
For a shorter time in the waiting room, please print and fill out the
New Patient Form and Release Form. If you were involved in an automobile accident, please print and complete our Motor Vehicle Accident Form
as well.
